Form to complete for required Community Service hours.

The form for students to use to log homework or communicate to teachers regarding an upcoming extended absence. 

Anyone under the age of 16 is required to have a work permit before being allowed to work in any job (with the exception of agriculture or domestic service). Employers must have a work permit on file before work begins.

Please review the work permit requirements to ensure you have all of the proper documentation to receive the permit. We are unable to issue work permits without all of the documentation.
